Transitional Case Management

After developing a transition service plan, the BSCC case managers continue to meet with each client at least weekly in ensure they are on track in meeting their goals. These meetings include a review of client’s progress, successes and challenges, and a discussion of strategies to address problems.  The case managers also meet with attorneys to ensure that each client is receiving appropriate legal services and that a continued presence occurs to ensure paperwork is submitted on time. 

Once clients have achieved sustainability and are ready to exit, BSCC assist them in securing permanent housing and making sure that they have a bank account and first month's rent and a security deposit.  The case managers conduct exit interviews, and monitor clients for six months before closing the case file.
